1. Identify the most recent evidence-based recommendation(s) for cardiovascular risk reduction related to lipids, diabetes, hypertension, obesity, etc.
2. Calculate the risk for cardiovascular disease using the 2013 ACC/AHA pooled-cohort data and determine appropriate pharmacologic interventions to reduce cardiovascular risk. 
3. Develop an interdisciplinary plan to implement CVD prevention counseling and follow-up.
