1. Counsel patients, using shared decision-making resources, regarding risks and benefits of prostate cancer screening.

2. Collaborate with other health providers to construct a coordinated referral process for men requiring prostate cancer treatment.

3. Develop a care plan to provide follow-up surveillance of the patient after treatment for prostate cancer.
