1. Develop a plan to routinely counsel patients on how to make healthy behavior changes to reduce their risk for developing hypertension or prehypertension.
2. Address barriers to care among patients in your practice, especially elderly and minority patients, and identify or develop tools to help manage hypertension.
3. Prepare treatment regimens including antihypertensive medications, lifestyle modification and available tools that may increase patient adherence.
