2. Calculate the risk for cardiovascular disease using the 2013 ACC/AHA pooled-cohort data and determine appropriate pharmacologic interventions to reduce cardiovascular risk.
3. Develop an interdisciplinary plan to implement CVD prevention counseling and follow-up.
